What you can do?

A referral is required to access Specialist Care.

 Complete referrals can be faxed to us at 403-221-4387 or submitted via Netcare eReferral.

 

We DO NOT need an MRI for referral

You may either send a referral to the next available surgeon or designate one

Fax to:  403-221-4387 or submit an eReferral through Netcare

 

Referrals will be triaged by our team.  We will send you and your patient a letter stating whether the referral has been accepted, and the approximate wait time.

 

We will contact your patient, when the time comes, with an appointment date and time.

  • Shoulder Consultation
  • Hip Preservation Consultation (Young Adult Hip)
  • Soft Tissue Knee Consultation

 

HOW TO REFER:

 

Please send a referral letter and recent imaging reports, please include:

 

  • Affected joint
  • Duration of symptoms
  • Whether there is pain with or pain at rest as well functional limitations, if any
  • Level of independence
  • Any walking aides
  • Walking tolerance (number of blocks)
  • Abnormal physical findings
  • Previous orthopedic surgeries
  • Significant medical conditions

At the Alberta Hip and Knee Clinic we provide consultation for 

hip and knee joint replacement surgery.

 

HOW TO REFER:

 

We accept referral letters or the AHS Hip and Knee Referral Form

 

If you send us a referral letter and recent x-ray report, please include:

 

  • Affected joint
  • Duration of symptoms
  • Whether there is pain with motion or pain at rest as well Functional limitations, if any
  • Level of independence
  • Any walking aides
  • Walking tolerance (number of blocks)
  • Abnormal physical findings
  • Previous orthopedic surgeries
